How to complete the entry form
  • Please write your name on the form as you wish it to appear in the programme, mark sheets and certificates
  • Please write your full address including the postcode and telephone number.
  • Where a teacher /school makes multiple entries, for your convenience you may complete one full entry form and then copy as many inside pages as required. Please then clip all the the additional sheets inside the full entry form. A separate form for each child or class is not required.
  • Entries will not be accepted without the correct entry fees or if they have not been signed by a responsible adult
  • Ages are to be calculated from the 1st September of the year preceding the Eisteddfod.
  • Fees are per entry not per person - eg only one fee is paid for an entry to a duet/trio class although there are two/three people performing.
  • Please ensure that your entry form is sent to the relevant secretary -  Music or Speech & Drama (names and addresses are on the forms and also below)
  • Please make sure you indicate on the entry form whether you will need the services of the official accompanist. Performers who require the official accompanist in "Own Choice" classes must make sure they send a copy of their music by the 1st February in the year of the Eisteddfod to:-

    Please Note: No reminders are sent and official accompanists reserve the right to refuse to play music which has not been received by the due date.
  • Copies of all Speech & Drama 'Own Choice' classes must be sent by 1st February in the year of the Eisteddfod to:-
       Speech & Drama - Miss Margaret Read, Ashgate Cottage, Rothwell, NN14 6HZ
  • Please ensure that you write clearly to avoid errors in the programme, mark sheets and certificates.
